Marco Fraccalvieri Marco Fraccalvieri, M.D. Via Baretti 4 10024 Moncalieri, TO, Italy Plastic Reconstructive and Aesthetic Surgery has two souls in one: a part designed to restore the anatomical and functional integrity of a part of the body that has gone damaged and another purely dedicated to improving the physiognomy. In the field of Aesthetic Surgery and Aesthetic Medicine, the principle that guides my profession is definitely "improving without upsetting": it is possible to obtain excellent results, which increase the pleasantness of the person, which make "feel better", which help to "stay better with themselves ", without exaggerating. In this way it is possible to improve the features of an individual without creating caricatures. The strength of Reconstructive Plastic Surgery is transversality. Plastic surgery is present in a capillary way throughout the diagnostic-therapeutic path of all pathologies that require a reconstructive need. Plastic surgery aims not only at the functional restoration of the lost anatomical part, but this concept is inseparable from the best achievable aesthetic result. The "difficult wounds" deserve a separate discussion in Reconstructive Surgery. In fact, most wounds have a rapid course towards healing; in some patients however, despite medical and nursing care, the course is particularly long and complex and does not always evolve into healing. In such cases, the injuries can go to serious complications or they can cause death. These injuries, consequently have a negative impact on physical and mental health and more generally on the quality of life of the patients, are the cause of prolonged hospitalizations and consumption of important resources, with significant repercussions on the health system in organizational and economic terms. Wounds that are difficult to heal can concern acute or chronic injuries of a varied nature and etiology, often in association with immune diseases, infections or chronic diseases. Giovanni Dapri is a Professor of Anatomy at the University of Mons (Belgium) and Professor of Surgery at the University of Brussels (Belgium). He is the Founder and Director of the International School Reduced Scar Laparoscopy. He is Course Chair of the different Laparoscopic Courses (Single Incision, Bariatric Surgery, Colorectal Surgery, and Abdominal Wall Surgery).
In his academic career, he has edited 5 surgical books, published more than 140 scientific articles and more than 80 book chapters. During International and National Congresses, he has performed more than 100 laparoscopic procedures in front of the audience in foreign countries and presented more than 450 oral presentations. He invented in 2008 an instrument set to operate in Single Incision Laparoscopy, and in 2013 an instrument set to perform the TransAnal Minimally Invasive Surgery. He is member of the Editorial Board of 49 Scientific Journals; member of Executive Committee of the American College of Surgeons (ACS), member of Scientific Committees of the Society of American Gastrointestinal and Endoscopic Surgeons (SAGES), and member of Scientific Committees of the American Society of Colon and Rectal Surgeons (ASCRS).
He is President of the American College of Surgeons (ACS) Belgian Chapter. He is a Faculty Member of the Courses held at IRCAD (France, Taiwan, Brazil), at LapSurg Institut (Sao Paulo, Brasil), at MISC-ASIA (Singapore) and at Cuschieri Skills Centre (Dundee, Scotland). I performed over 2500 surgical procedures as a primary surgeon. Among these, about 400 liver transplants (adult, pediatric, split, and re-transplants) and over 150 kidneys, pancreas, small bowel, or multivisceral transplants. I performed more than 200 major hepatobiliary procedures (open, laparoscopic, or robotic).
I pioneered the use of very old donors in liver transplantation (reporting the first series of nonagenarian grafts), and the use of ex-situ machine perfusion and DCD donors in Italy. I was solely responsible for starting an FY1 teaching program for the department of general surgery in Stepping Hill hospital, Stockport, UK.
I was also involved as Faculty in the Manchester ASiT MRCS course in September 2018.
I conducted weekly tutorial classes and weekly clinical case discussions for post-graduate students for a period of 1 year during the 3rd year of residency in General Surgery in India as well.
I also undertook informal teaching sessions of F1 doctors whenever possible during my term working in NHS.

- Interventional therapy for digestive tumors: I have engaged in manufacturing the arterial embolism agent as a assistant research fellow from Jul.1994 to Aug.1997. I have successfully manufactured the degradable starch microspheres(DSMs) with the help of physical chemist for 3 years .As a new degradable embolism agent, can be used to treat about digestive tumors such as stomach, liver and intestine.
- Pathogenesy of acute liver failure: I have engaged in the basic research for acute liver failure as a assistant research fellow from Jul.1997 to Aug.2000.nitric oxide(NO) , as a preinflammatory factor and signal transduction messenger , plays an important role in acute liver failure after partial hepatectomy , especially participating in multiple organ dysfunction such as liver, lung, kidney, intestine etc . By interfering with NO production , MODS can significantly be prevented or decreased.
- Signal transduction pathway of hepatocellular carcinoma: I have engaged in signal transduction pathway of tumor as associate investigator from Aug.2000 to Sep.2003.Signal regulatory protein alpha(Sirpa), as a inhibitor factor and member of signal regulatory proteins, is involved in liver regeneration after partial hepatectomy, mainly participating in liver regeneration termination . Sirpa is associated with occurrence and development of hepatocellular carcinoma , the low expression of Sirpa is contributive to occurrence and development of hepatocellular carcinoma by increasing the expression of nuclear factor ?B (P65 and P50). now the correlation between Sirpa and immune rejection is being done a research after liver transplantation.
- Drug targeting therapy for hepatocellular carcinoma: I have engaged in manufacturing the drug targeting agents as research fellow from Aug.2000 to Sep.2003.I have successfully manufactured the immune nanoparticles of brucine , as a target therapeutic drug , induced apoptosis of liver cancer cells and inhibited the growth of liver cancer cells by high protein and mRNA expression of Fas . it is potential development drug for hepatocellular carcinoma .
